Extreme Aquatic Team
Want to give us a try? It’s easy. There is a 2-week trial
period for the SR program (13 & up.) There is a 1-week trial
period for the AG, NEAT, and L2SC programs (12 & under.) Make a
check payable to EAT in the amount of $51 to cover USA Annual
Registration fee. For insurance purposes, these forms and money are
required before your child can enter the water. If your child
decides not to join the team at the end of the trial period, your
check will be refunded to you. If your swimmer swims beyond the
trial period, the USA Registration check will be cashed, and
monthly fees will be charged accordingly.
1. Complete the USA Swimming Athlete Registration Application at
the pool. Make a check payable to EAT in the amount of $51.
2. Complete and return the General Information and Medical
Information and Release Form.
3. An established EAT parent will explain and complete the EAT
Enrollment Contract with you at the pool. It must be signed and
returned with a copy of your child’s birth
certificate.
4. Sign and return the Enrollment Contract Membership
Requirements.
This will only cover monthly dues and water fees. Swim meets and
equipment fees are paid separately.

Policy
There is a two
week trial period for the Senior swimmers (13 & over,) and a
one week trail period for the Age Group, NEAT, and Learn-to-Swim
Competitively swimmers (12 &under). If your swimmer swims
beyond the trial period, the USA Swimming Application check will be
cashed and fees will be charged accordingly.
