2009 Fall Stroke Clinic

 
The Lakeside swim team is proud to present our annual Stroke Clinic for the fall of 2009!  For our flyer about our Stroke Clinic click here.  Our first day will be Saturday, September 19th.  We will offer classes from 3:00-4:00 PM, and from 4:00 to 5:00 PM.

The $180.00 check can be mailed with the form or can be turned in on the first day of Stroke Clinic.  Please try to have the forms turned in as soon as possible as the classes are filled first come, first serve.  We will be accepting forms as late as the first day of stroke clinic but to insure a spot in the clinic, it is recommended to have the forms turned in ASAP.  Looking forward to seeing you in our clinic!
