Registration Forms

To register for the team, you may download and print the following forms below:

  • 2009 Registration Form (available in both a print-only and fill-in version)
  • 2009 Swimmer Emergency Card (available in both a print-only and fill-in version)
  • 2009 Parent Code of Conduct
  • 2009 Swimmers Code of Conduct

Bring completed paperwork, along with payment, to the Ice Cream Social on March 6 at the Clubhouse from 7 to 8 pm.  OR, mail or drop off registration materials to Simon Newbigging, 4481 Oakbrook Ct., Concord, CA 94521.
