Registration Packet

If you are interested in joining SAC, please click on the button below to download and print the Registration Packet.  There are several forms included in the packet that will need to be completed, signed, and turned into the SAC Office prior to joining, such as the Registration Form, Photo Release Form, Understanding of the Parent Volunteer and Fundraising Commitment Form, and the Code of Conduct.   To schedule a try-out, please contact the Office Administrator at Cactus Pool (480) 951-5368.
