Yearly Fees

 
MAC requires yearly fees for every swimmer. 

Your yearly fees include the following:

1 team shirt, 1 team cap, USA swimming fees

1. Attach a copy of the following forms filled out:

MAC registration form

MAC medical form

USA swimming athlete form

Copy of your swimmers birth certificate

Bring all forms with your payment of $100.00 made to EVCF to practice and give to your coach.

If you are transferring from another team fill out the athlete transfer form.
