Listed below are the HYCAT Registration Forms for
the 2009-2010 HYCAT Swimming Program. There are two (2)
forms: [1] the first form contains the HYCAT registration
information that is required by the HYCAT Swimming Office
and financial information for the 2009-2010 HYCAT Swimming
Program; and [2] the second form contains information regarding the
various HYCAT Swimming
Teams/Programs.
Please complete all of the information requested
and return to the HYCAT Swimming Office within 10 days after
joining the HYCAT Swimming Progarm. [NOTE: you will need
long paper to print any of the forms listed
below].

Listed below are the USA Registration Forms for
the 2009-2010 HYCAT Swimming Program. There are three (3)
forms: [1] for any swimmer who joins the HYCAT Swimming Program
from 09/01/09 thru 04/30/09 and ALL HYCAT "Competitive" Team
members; [2] for any swimmer who joins the HYCAT Swimming Program
from 05/01/09 thru 08/31/09; and [3] for any non-swimmer/athlete
who serves as a member of the HYCAT Coaching Staff; HYCAT Board
members and/or HYCAT stroke & turn
officials.
Please complete all of the information requested
and return to the HYCAT Swimming Office within 10 days after
joining the HYCAT Swimming
Progarm.
