Initial Team
Registration Form- Please print out this form
and complete all information. Submit the registration form to
the Front Desk Staff at the Pool and Tennis Center along with the
required fees.
Parents
Board Expense Report- Please use this form to
submit any expenses incurred on behalf of the Cyclones for
sanctioned events, items as approved by the Parents
Association. Please submit to Marcie Martine directly or they
can be left at the Front Desk.
Accounts
Payable Form - Please use this form to submit any vendor invoices to
the Parents Board Treasurer for all approved purchases by the
Parents Association. Please submit to Marcie Martine directly
or they can be left at the Front Desk.
Handbook
Sign-off Form - Please print and sign this form to
confirm that you have read and understand the policies outlined in
the Cyclones Parents Handbook. Please turn in form as
indicated at the bottom of the form.
Volunteer Hour
Request Form - Please use this form to submit for Volunteer Hours that you worked at an event and did
not sign up for online to ensure you get credit for those hours
worked. Please turn in
the form to the Volunteer Hour Coordinator as indicated at the top
of the form.
Mileage Request
Form- This form
is for coaches ONLY! Use this form to submit
for mileage reimbursement
for the use of your personal vehicle to attend meets outside of the
local county area (Dade, Broward).
