Forms to fill out and bring to your child's in-water swim skill assessment

 
Please fill out the WAVES New Swimmer Info form and give it to the coach at your child's in-water swim skill assessment.

If coming from another USA Swimming club, please bring a copy of the Club Transfer form.  The coach will help you complete the form with correct information.  You will then mail the form with payment to the Illinois Swimming office.
