Held under sanction of USA Swimming, Inc.
Liability: It is understood and agreed that USA Swimming, Inc.,
Arizona Swimming, Inc., Surprise Storm and the City of Surprise,
and all meet officials shall be held harmless from any and all
liabilities or claims for damages arising by reason of injuries to
anyone during the conduct of this meet, which includes warm-up
sessions.

Meet Director: Tim Manley
Course: Outdoor, 25 yard, 8 lane heated pool, Colorado Starting
system, electronic timing. Warm-up and warm-down will be available
in the diving well. Arizona Swimming warm-up/warm down procedures
will be posted and enforced.
Eligibility: Open to any Arizona Swimming registered swimmer
training with or training unattached with Surprise Storm(S2),
Sunwest Swimming(SWS), Vitras Swim Club(VSC), Arizona Swimming
Gauchos(ASG) and Anthem Dolphins(AD), holding a current USA
Swimming registration card as of the meet entry deadline.
Please explain this to your parents / athletes when you send this out! Event’s 3-12 and Event’s 27-36 will be combined. They are like one event, this allows for them to choose the 200 event they wish to swim at that time. If they enter two events I will pull the 2nd one out. Hopefully they do not.
Rules: 1. Events are pre-seeded, timed final events, heat winner
awards only.
2. Swimmers are limited to 4 individual events per day.
3. To enter, individuals must submit fastest conforming (SCY) timed
achieved.
4. No time trials offered other than those scheduled within the
meet events.
5. Swimmers sessions may change and be notified as soon as
practical after the meet entry deadline.
6. Entries will be limited to conform with local gathering
limits.
Awards: Heat winner awards only.
Entry Fees: Individual Events: $3.00
LSC Surcharge $8.00
Assumption of risk disclaimer statement:
An inherent risk of exposure to COVID-19 exists in any public place
where people are present. COVID-19 is an extremely
contagious disease that can lead to severe illness and death.
According to the Centers for Disease Control and Prevention,
senior citizens and individuals with underlying medical conditions
are especially vulnerable.
USA Swimming, Inc., cannot prevent you (or your child(ren)) from
becoming exposed to, contracting, or spreading COVID-
19 while participating in USA Swimming sanctioned events. It is not
possible to prevent against the presence of the disease.
Therefore, if you choose to participate in a USA Swimming
sanctioned event, you may be exposing yourself to and/or
increasing your risk of contracting or spreading COVID-19.
BY ATTENDING OR PARTICIPATING IN THIS COMPETITION, YOU VOLUNTARILY
ASSUME ALL RISKS
ASSOCIATED WITH EXPSOURE TO COVID-19 AND FOREVER RELEASE AND HOLD
HARMLESS USA SWIMMING AND ARIZONA SWIMMING AND EACH OF THEIR
OFFICERS, DIRECTORS, AGENTS, EMPLOYEES OR OTHER REPRESENTATIVES FRO
MANY LIABILITY OR CLAIMS INCLUDING FOR PERSONAL INJURIES, DEATH,
DISEASE OR PROPERTY LOSS, INCLUDING BUT NOT LIMITED TO CLAIMS OF
NEGLIGENCE AND GIVE UP ANY CLAIMS YOU MAY HAVE TO SEEK DAMAGES,
WHETHER KNOWN OR UNKNOWN, RORESEEN OR UNFORESEEN, IN CONNECTION
WITH EXPOSURE, INFECTION, AND/OR SPREAD OF COVID-19 RELATED TO
PARTICIPATION IN THIS
COMPETITION.

established by the CDC, USA
Swimming, State of Arizona Executive Order, and Arizona Department
of Health Services (AZDHS).

Covid Protocol: Surprise Storm will be using the following rules
to allow for a safe / enjoyable environment.
Ï Athlete / Parent Entry with a mask – 1 parent may
cheer on the turn end of the pool for athlete, both athlete and
parent must exit through the side gate following the athletes
event.
Ï Team “camp” area on the north side of the
aquatics center. Please maintain social distancing protocols.
Ï Failure to comply to covid protocol may cause dismissal from
the event by the meet director.
