  1. Compile your registration materials:
    1. Registration Form: One per family. If you have more than three swimmers, you will need extra forms. Only the swimmer information needs to be listed on the extra forms. This document provides us with all the information we will need to contact and register your swimmer with USA Swimming. Please be sure to fill it out completely and have all parents/guardians as well as all swimmers sign the code of conduct agreement. Please mark clearly the swimmers practice group, practice location and which sessions they will be registering for.
    2. Medical Treatment Authorization and Consent: One per swimmer. Whether at a practice, meet or on a team trip we want to make sure that your swimmer is safe. This form will help us to assure that your swimmer is safe and cared for at all times.
    3. Financial Assistance: If you will be applying for financial assistance, you will need to complete this form and schedule a time to meet with Brent Graham before you register. Contact him at at 816.858.0114 or brentgraham@kansascityymca.org to schedule a time.
    4. Payment:
      1. Practice Fees: Practice fees can be paid in a lump sum at the time of registration or can be paid with a monthly draft. If paying a lump sum, you can pay with a card, check or cash. If paying with a monthly draft, you can pay with a card or using a bank account. To pay with a card, we will need to save the card to our system. To draft from an account, we will need a voided check for a checking account or a letter from your bank confirming the routing and account numbers for a savings account.
      2. Registration Fees: Registration fees will be collected at the time of registration. To collect these fees we can use a credit or debit card on file, the one for practice fees can be used, or you will need to have cash or a check to make this payment.

We have many options to choose from for the swimmer or the whole family. Let us know what works for you. Remember that your Financial assistance can help with a membership too.
