Michigan Swimming, Inc. COVID-19 Update: MDHHS Rapid Test Ordering Information

Club Presidents and Head Coaches,
 
The Michigan Department of Health and Human Services released details of the youth sports COVID-19 antigen test program this afternoon, including details on how to secure free COVID-19 antigen tests. While Michigan Swimming is still working through the documents, I wanted to pass the information along to you as soon as possible.

The application to receive free COVID-19 antigen tests from MDHHS is available here. Each club must submit their own application.
 
Per MDHHS, you must certify that you have the following as part of your application:
 
  • Clinical Laboratory Improvement Amendments (CLIA) Waiver (form here; process here; waivers are facility-specific)
 
  • Personal Protective Equipment Plan
 
  • Biohazard Disposal Plan
 
  • Certification of Testers Watching Training Videos (videos here and here)
 
  • Ability to Comply with Reporting Requirements
 
Once you have submitted your application, you will be provided the link to order COVID-19 rapid antigen tests from MDHHS. Save this link for future use. Only clubs who have completed the application will be able to use this link.
