Here is everything you will need to register your swimmer(s) for the 2021 Monarchs Swim Team...

(Please Note: We are a neighborhood swim team - you must be a member of Kingsgate 3 & 4 in order to be a member of the Kingsgate Monarchs Swim Team.)

1. Print the   Cover Sheet for directions and the 2021 fee schedule.
2. Print & fill out the   2021 Swim Team Registration Form.
3. Print & fill out the KGM Covid Waiver 
4. Print & fill out the  Midlakes Hold Harmless Agreement.
5. Print & fill out the  Concussion Information Sheet (your swimmers must also sign this).
6. Send - or drop off - your payment with all completed forms to:
