Middlesex Swimming Vaccination Passport Policy 

Effective Wednesday, September 22nd, 2021

 

To further protect people in Ontario as the province continues to confront the Delta-driven fourth wave of the COVID-19, the government, in consultation with the Chief Medical Officer of Health, will require people to be fully vaccinated (two doses plus 14 days) and provide their proof of vaccination along with ID to access certain businesses and settings starting Wednesday, September 22, 2021

With this announcement The Club Vaccination Passport Policy will include facility by facility Policy's, as each facility that we train out of will be taking a different approach to these new health measures being put into place by the Ontario Government in addition to our own Club Policy.

 

MIDDLESEX SWIMMING CLUB POLICY

  • Effective Wednesday, September 22nd, 2021 anyone 18 years or older regardless of what program they are participating in will be required to provide proof of vaccination in the form of a printed version of the e-receipt individuals receive upon their 2nd dose accompanied by a piece of ID that includes name and date of birth (photo ID not required currently). 
    • No data will be collected, tracked, or archived and individuals will need to show proof each time they enter.
  • As of Sunday, October 31st, 2021 The Club will require all members ages 12 and older (2009
    DOB)
     to be fully vaccinated (two doses plus 14 days) and provide their proof of vaccination along with ID to access our program at all facilities within London, St. Mary's, or Stratford, if not already required at each respective facility.
  • Rare exception will be provide to those individuals who cannot be vaccinated due to permitted exemptions (medical and other protected grounds under the Ontario Human Rights Code).  
  • The Club cannot go against any Policy put into place by a respective facility. Therefore each facility Policy's supersedes The Club Policy no matter how strict the Policy may be. 

 

WHAT IS PROOF OF VACCINATION?

The Ontario Government has provided a detailed description of what will be accepted as proof of vaccination. A person will be considered fully vaccinated if they have received:  

  • The full series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines, or
  • One or two doses of a COVID-19 vaccine not authorized by Health Canada, followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada, or
  • Three doses of a COVID-19 vaccine not authorized by Health Canada; and
  • They received their final dose of the COVID-19 vaccine at least 14 days before providing the proof of being fully vaccinated.

 

WHAT IS PROPER PROOF OF IDENTIFICATION?

In order for an individual to verify proof of vaccination, they must provide the receipts documenting receipt of the vaccines and identification that provides the individual’s name and date of birth that matches the vaccine documentation. Photo documentation is not required. Examples of the types of identification that are acceptable include:

  • A birth certificate;
  • A citizenship card;
  • A driver’s license;
  • Government (Ontario or otherwise) issued identification card;
  • An Indian Status Card/Indigenous Membership Card;
  • A passport;
  • A permanent residence card.

 

NON-COMPLIANCE WITH  POLICY'S

Non-compliance with the requirements of The Club Policy or any facility policy can trigger substantial risk to our staff, our members, their families, and the community at large. Accordingly, and in line with our policies, directives and applicable legislation, any non-compliance with such policy's may result in discipline up to and including termination of employment (for staff), termination of engagement (for volunteers), or termination of membership (for athletes).

FACILITY VACCINATION PASSPORT POLICY'S

 

City+of+London

CITY OF LONDON 

  • Effective September 22nd, 2021 anyone 18 years or older regardless of what they are doing in the facility will be required to provide proof of vaccination each and every time they enter the facility.  
  • Effective September 22nd, 2021 anyone 17 or under who is actively participating in a registered program, club practice, or public drop-in program will NOT need to show vaccination status yet.  
  • Effective September 22nd, 2021 anyone 12 or older who is spectating, accompanying others or participating in dry-land training using weights or exercise WILL need to show proof of vaccination. 
  • Patrons who provide written document completed and supplied by a physician or registered nurse stating that the individual is exempt for a medical reason from being fully vaccinated against COVID-19 and the effective time-period for the medical reason are also exempt.
  • Children who are 11 years of age and younger and unable to be vaccinated will also be exempted from these requirements.
  • Proof of vaccination required would be a printed version of the e-receipt individuals receive upon their 2nd dose accompanied by a piece of ID that includes name and date of birth (photo ID not required currently). 
    • No data will be collected, tracked, or archived and individuals will need to show proof each time they enter.

Boys+%26+Girls+Of+London

BOYS & GIRLS CLUB OF LONDON

  • Effective September 22nd, 2021 anyone 18 years or older regardless of what they are doing in the facility will be required to provide proof of vaccination each and every time they enter the facility.  
  • Effective September 22nd, 2021 anyone 17 or under who is actively participating in organized sport will NOT need to show vaccination status yet.  
  • Patrons who provide written document completed and supplied by a physician or registered nurse stating that the individual is exempt for a medical reason from being fully vaccinated against COVID-19 and the effective time-period for the medical reason are also exempt.
  • Children who are 11 years of age and younger and unable to be vaccinated will also be exempted from these requirements.
  • Proof of vaccination required would be a printed version of the e-receipt individuals receive upon their 2nd dose accompanied by a piece of ID that includes name and date of birth (photo ID not required currently).

Town+of+St.+Mary%27s

TOWN OF ST. MARY'S

  • Effective Wednesday, September 22, 2021, patrons 18 years of age and older wishing to access the Pyramid Recreation Centre, Aquatics Centre and Friendship Centre for programs and recreational services will need to show proof of being fully vaccination (two doses plus 14 days) along with ID upon entry.

  • Effective Sunday, October 31 2021, all patrons 12 years of age or older will be required to show proof of vaccination prior to entry, subject to Provincial exemptions.

  • Individuals who cannot receive the vaccine due to medical exemptions will be permitted entry with a doctor’s note and a negative COVID-19 test that is no older than 48-hours old until recognized medical exemptions can be integrated as part of a digital vaccine certificate.

  • Anyone wishing to access the Pyramid Recreation Centre for ice or pool programs must use Entrance B.

  • Those needing to access the Friendship Centre must continue to use Entrance C.

 

Western+University

WESTERN

  • Full vaccination against COVID-19 is required for all individuals involved in any in-person activities on campus (students, staff, faculty, contractors, and visitors),
    • with the rare exception of those individuals who cannot be vaccinated due to permitted exemptions (medical and other protected grounds under the Ontario Human Rights Code).
  • All individuals who are unvaccinated due to permitted exemptions or who are awaiting their second dose be required to adhere to additional health and safety measures, up to and including serial and frequent rapid testing.
  • Individuals involved in any in-person activities on campus (students, staff, faculty, contractors, and visitors) should be fully vaccinated as soon as operationally feasible.
  • Proof of vaccination required would be a printed version of the e-receipt individuals receive upon their 2nd dose accompanied by a piece of ID that includes name and date of birth (photo ID not required currently). 
    • No data will be collected, tracked, or archived and individuals will need to show proof each time they enter.
