Should you have any questions, or to submit your forms, please contact our Club Officials Chair, Laurie Krawczuk (laurie.slsc@gmail.com).

An official agrees, by submitting their application to be a registered Swimming Canada / Swim Ontario official, to abide by and comply with the By-Laws, policies, procedures, rules, and regulations of Swim Ontario including the Swim Ontario Comprehensive Code of Conduct and Ethics.  They fully understand the implications of being a registrant of Swim Ontario.

The Club Officials Administrator (COA) will initiate the registration once they receive the Privacy Consent Form (PIPEDA) form from each official in their club.
