2018 Spring/Summer Registration - Policies

Please read and following instructions below regarding HHSC required and informational documentation. The required documents must be completed, signed and on file with HHSC in order for the registration process to be complete for your swimmer(s). Your swimmer(s) will not be able to participate with HHSC until all required documents have been submitted.

REQUIRED

Please complete and sign one of the two USA Swimming Athlete registration forms to the right for each swimmer. Most members will complete the first form but if any of the following criteria apply to you, please complete the "Outreach" form instead:
The following documentation is acceptable as proof for Outreach Fees and will need to be submitted at the time of registration:
1. Free or Reduced school lunch program documentation. School letter sent home to family upon school corporation application process or email from school administration is acceptable.
2. Hoosier Healthwise Insurance card copy
3. WIC card copy
4. Other forms accepted if presented and would be equivalent to above​

REQUIRED Please complete the Swimmer Information section on page 1 and read pages 2-4 of the Membership Contract. Initial each section as required and sign and date on the last page.  An HHSC staff member will complete the  remainder of the first page to include the fee information. REQUIRED for New Swimmers All new swimmers are required to submit a copy of their birth certificate to complete their registration. Birth Certificate
