LOA and Voluntary Termination

O2 Performance Aquatics understands that there are times when a family will not participate for an extended period of time due to family obligations, illness or injury.  Each family must understand too, that O2 cannot operate financially on a "drop-in" basis.  Monthly income must be consistent for us to pay our coaches and meet our other obligations.

Medical Leaves of Absence:  Medical leaves require a note from a medical doctor to waive monthly dues up to 90 days. If the medical leave is 91 days or more, the member must pay the annual membership fee to rejoin. Medical leave does not require a 30 day notice.

Leaves of Absence (LOA) leave of absence and suspension of monthly dues will be allowed under the following conditions:

  1. The duration is no less than 30 days and no more than 90 days;
  2. Dues are current before the actual start date of the leave; and
  3. All Fees & Obligations are in good standing (youth team only)
  4. The member agrees to pay a $15/month account maintenance fee.

Notice must be given to the Treasurer in written form 30 days prior to the leave start date (email is acceptable - use the Contact Us option on the website).

Voluntary Termination:  If a member voluntarily leaves the team for any reason at any time of the year, a 30 day notice must be given to the Treasurer in written form (email is acceptable - use the Contact Us option on the website). If no notice is given, one month's membership dues will be assessed. Account holder for that member is responsible for any outstanding balance upon termination. Unpaid amounts will be considered delinquent if not paid by the last day of the month in which it is due, regardless of continued membership with O2. O2 reserves the right to notify any and all other USA swim teams of an account holder's delinquent account.
