Please complete and return this registration form for your child's/children's session. The FIRST SESSION MUST BE PAID IN ADVANCE. You are welcome to reserve up to two additional sessions without payment to hold your reservation. Payment is due one week in advance to the first day of the session. Registration and Insurance fees are non-refundable.
