COVID-19 Vaccination Update
As you have probably heard, the Province of Nova Scotia is instituting a poof of vaccination policy. People who are 12 years old and older will need to provide proof of vaccination as a participant or spectator in indoor and outdoor sports practices, games or competitions/tournaments as of October 2, 2021.
The following are important additional details from the Province:
- Newly eligible 12-yr-olds - those who turned 12 between Jan. 1, 2021 and Oct. 4, 2021 - will have until December 31, 2021 to attend events and activities while they get vaccinated.
- Children who turn 12 after Oct. 4, 2021 will have three months from their birthday to provide proof of their second vaccination.
- 13 to 18 year olds who can provide proof of 1 dose can participate in sport, recreation, arts and culture programming hosted by a business or organization. To continue participating, you must provide proof of full vaccination by Nov. 9, 2021.
All swimmers currently attending practice have met the provincial guidelines. If you are new to the team or if your swimmer is age 12 and getting vaccinated, you will be asked to email proof of vaccination to [email protected] along with the consent form below. Emails will be deleted within 24 hrs and we will only store the fact that we have received documentation. The fact that we have verified proof of vaccination for your swimmer may be shared with aquatic facilities where your swimmer's practices are located to facilitate their entry into the facility.
I, ___________________ (insert swimmers name), hereby give Swim Nova Scotia and Halifax Trojan Aquatic Club permission to collect and use my personal health information, specifically my COVID-19 vaccination status and record verification in the Swimming Canada Registration database. This information will be used solely for the purpose of determining and verifying my eligibility to access and/or participate in non-essential services and activities in accordance with the COVID-19 Protocol for Proof of Full Vaccination for Events and Activities set out in the Nova Scotia Public Health Order, and will not be otherwise collected, used, or disclosed without my approval.
