Thank you for joining FORM Swim Team.

Here are a few items to know prior to registering

• All registrations need to be completed by the parent, unless swimmer is 18 or older.
• There is a $45.00 registration fee for each swimmer who joins the team. The fee includes a team shirt and access to the team website.
• Make sure to include your swimmer's t-shirt size when registering. The team uses this selection when ordering your swimmer's shirt.
• Please read over the team handbook. This includes important information about the team and can be found under the Documents section on the team website.

Once your account has been approved, you will receive one or both of the following emails

• Welcome email, which includes our Welcome Letter and 2023 Long Course Handbook
• New Families will receive an Account Approval Email used to finish setting up NEW accounts

At that time you can setup Automatic Payments, view your swimmers best times, declare for meets, update contact information, and more.
