USA Swimming Transfer Form
This form is for swimmers that are transferring from or to a USA swimming club. This form needs to be downloaded and filled out and sent to the Illinois Swim Office.
USA Swimming Athlete Registration
This form is for swimmers that are registration for current athlete registration with Illinois Swimming. This is required for all current athletes participating in practices and meets.
This form is required for all members. This form needs to be downloaded and signed if not already completed.
