This registration form is for Our Lady of Mercy students enrolling in the CA Middle School Training group program, August 21-October 24.

*IMPORTANT- if you are a student participating on your middle school team but NOT training with the CA MS Training group (ie, you swim in a year round CA or TAQ group, or elsewhere) you do not complete this form.  Your school rep will collect your meet and insurance fees for league particiaption separately.

This form is only for individuals practicing with the CA Middle School training groups.

Fees you will pay with your online registration:

• $125 training group tuition 
• $10 MS League Insurance fee
• $32 Season meet fees

Season Information:

• Practices timeslot assigned to OLOM: 5:30-6:30pm, Mon-Thurs
• Practice dates:  August 21-October 24 (no practice on Mon, Sept 2)
• Practice location:  Crawfish Aquatics (Bluebonnet), outdoor pool.  **we move inside on rainy days!**
