Carpool Permission Form: This form is needed for all minor athletes traveling to practice or team functions with another 18+ athlete.
Competition Permission Form: This form is needed for all minor athletes traveling to competition with another 18+ athlete.
Lodging Permission Form: This form is needed for all minor athletes lodging with another 18+ athlete for the purpose of training or competition. The form needs to be completed prior to travel.
