Tennessee Concussion Youth Sports Information
Required for ALL KISL Coaches:
1. Download the Information and Signature Form for Coaches here
- Read and keep the information.

2. Complete the NFSHSA concussion free video course at nfhslearn.com
- Find the Free / Elective Online Courses
- Find the course: “Concussion in Sports”
- Take the course, download the certificate
- Submit to the KISL form: https://docs.google.com/forms/d/e/1FAIpQLSceFZn-Rs3V6eNjLLuoJW4Q3knyvr4TBMChvhe6G8tgYGspSQ/viewform?usp=sf_link
All coaches must complete these forms every year prior to assuming any on-deck coaching activities. If a coach has already completed this process for another sports team, that coach may submit a copy of the signed signature form and certificate to KISL to satisfy certification requirements.
KISL is required to keep copies of proof of certification from EACH coach every year for three years, as stated by Tennessee State Legislation Public Chapter 148.

- Read and keep the information.
- Sign the signature page and give to team coach or other contact person.
2. Optionally the Parent and/or Athlete may view the NFSHSA concussion free video course at nfhslearn.com

- Find the course: “Concussion in Sports”
Each Parent/Athlete is required to complete a new signature form every year.
Each team is required to keep copies of proof of certification from EACH parent/athlete every year for three years, as stated by Tennessee State Legislation Public Chapter 148.
