Medical Release Waiver

I certify that I am the parent or legal guardian for my child(ren). I hereby agree and authorize any employee or agent of the Santa Teresa Sea Otters, Inc. or Rancho Santa Teresa Swim & Racquet Club to have the swimmer(s) treated in any medical emergency while the swimmer(s) is/are participating in the program. I understand and agree to pay all costs associated with medical care and transportation for the swimmer(s).

 

I hereby waive, release and forever discharge Santa Teresa Sea Otters, Inc. and Rancho Santa Teresa Swim & Racquet Club and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Santa Teresa Sea Otters activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all Swim Team activities.

 

Liability Waiver

WAIVER/RELEASE OF LIABILITY -- PLEASE READ CAREFULLY BEFORE SIGNING THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS

 

By registering my child(ren) with the Santa Teresa Sea Otters, Inc. I agree and understand that swimming is a HAZARDOUS activity. I recognize there are risks inherent in the sport of swimming, including but not limited to paralyzing injuries and death.

 

As the parent/guardian of the swimmer(s), I hereby agree to allow the swimmer(s) to participate in the Santa Teresa Sea Otters Swim Team (“the program”) and hereby agree to indemnify and hold harmless Santa Teresa Sea Otters, Inc., and its coaches, board members, officers, directors, agents and employees against any liability resulting from any injury that may occur to the swimmer(s) while participating in the program. I further agree to indemnify the Rancho Santa Teresa Swim & Racquet Club, and its board members, officers, directors, agents and employees from any damages incurred arising from any claims, demands, actions or causes of actions by the swimmer(s).

 

Photo Release

By registering for the Santa Teresa Sea Otters swim team, the athlete or his/her guardian consents to the publication, via electronic, print or other media, information and/or imagery related to their participation this season. Such information and imagery may include, but is not limited to, photographs, video, newsletters, team website, social media, print media, heat sheets, and live or online meet results.

All photographs must observe generally accepted standards of decency in particular:

  • Action shots should be a celebration of the sporting activity and not a sexualized image in a sporting context.

  • Actions shots are NOT to be taken or retained where the photograph reveals a torn or displaced swimsuit.

  • Photographs are NOT to be taken from behind swimming blocks at the start of a race or exhibit a child climbing out of the swimming pool.

  • Photographs are NOT to be taken in locker rooms or bathrooms.

 

 

Conduct Policy Agreement

In order to be an active member of the Santa Teresa Sea Otters Swim Team, I promise to act in accordance with the Sea Otters mission statement and abide by all the rules and regulations of the club, to attend swim practice and meets regularly, to put forth a conscientious effort to continually improve my swimming ability, and to exhibit good sportsmanship at all times. I understand that disruptive or disrespectful behavior will not be tolerated. Coaches and Board Members have the authority to remove a disruptive or disrespectful swimmer or parents from practice, meets, or team events. Multiple behavioral issues may result in additional action, as determined by the board, which may result in suspension or dismissal from the team with no refund of fees.

 

  1. All Rancho Santa Teresa Swim and Racquet club dues and Associate Member dues will be current before the first day of practice or swimmers will not be allowed to participate.
  2. I agree to attend as many practice sessions as possible. USA Swimming and/or high school swimmers may arrange with the coach to practice with their USA Swimming and/or high school team.
  3. I will conduct myself in a respectful manner to parents, teammates, coaches and officials.
  4. I will notify the Head Coach by Wednesday, prior to a meet, if I cannot attend the meet on Saturday. I realize failure to do so may result in a loss of relay privileges.
  5. If I am scheduled to swim in a relay, I will notify the Head Coach by Wednesday, prior to a meet, if I cannot attend the meet on Saturday. I realize failure to do so may result in suspension or dismissal from the team.

Additionally:

  • Parents/Guardians of the swimmer(s) are physically and financially responsible for their swimmer's behavior and actions while participating in any League sponsored activity.  Any non-participating guest at a league event is expected to follow the same code of conduct and will be the responsibility of the adult who brought the minor to the activity.

  • Dishonesty, theft, and/or vandalism will not be tolerated.

  • All swimmers and parents will follow photography League by-laws. When posting photos from League events to social media the utmost respect and consideration will be used.

Violating any League code of conduct bylaw may result in a League consequence. If the violation occurs during Champs or another League sponsored event, the consequence will be voted on by the League Board. If the violation occurs during a dual meet, the two teams involved will coordinate resolution of the issue.

Parent Volunteer Policy

Parent volunteers are essential to the success of the Sea Otter Swim Team. To run the swim meets and our social events effectively, we must have volunteers.  Volunteer shifts are fun and very community oriented, but require a significant commitment from our swim families. At every meet your swimmer plans to attend, including Time Trials and Champs, you should be prepared to work at least one shift.  At any meet it may be necessary to work an additional shift or cover an open shift, sometimes at short notice.

 

Our policy is outlined below. Each family will be required to sign up for:

1. Swim Meets (including Time Trials):         5 shifts

2. Sea Otter Events: 1 shift

3. Championship Meet:                    1-2 shifts

 

Volunteer signups will open on March 18, 2020 and are on a first come, first serve basis.  You will have until April 1, 2020 to complete all of your volunteer signups (excluding Champs -instructions will be sent later in the season for the Champs meet).  Please note, your registration will not be finalized until you have signed up for all of your volunteer shifts for the season (excluding Champs). Any family who has not fulfilled their signups will be assigned shifts by the Volunteer Coordinators. While we appreciate that not everyone knows their summer plans, we ask that you please do your best.  By signing up you are not guaranteed that shift, but we will do our best to honor your specific requests. We may need to make some adjustments to fill all positions, as needed. Please sign up for only one shift per meet, unless you know there is a meet you will be missing.

A credit card authorization, in the amount of $50 per meet, for a maximum of $400, (per family), will be required in order for your registration to be complete, due on registration.  If for any reason you cannot work your shift, it is YOUR responsibility to find your replacement and notify the Volunteer Coordinators of the change. $50 per shift will be deducted from your deposit for the following reasons:

    -You fail to show up for a shift

    -You fail to notify the Volunteer Coordinators by Wednesday, at 9pm, prior to the shift to be missed AND you do not find a replacement for yourself.

By registering for the Santa Teresa Sea Otters swim team, I hereby authorize the Santa Teresa Sea Otters to charge my credit card $50 (plus processing fees) if I fail to meet my volunteer obligations as outlined above.
