Important Information about LEAVING SUSA Swim Team

• Swimmers that decide to leave the team must notify SUSA Billing Admin by filling out the 30-Day Notice of Cancellation form (link below).  Telling your coach or just not showing up does NOT constitute notice.
• Departing members are obligated to pay any outstanding monies owed to SUSA Swim Team. 
• Dues will continue to accrue until SUSA Billing Admin is notified that the swimmer is planning to leave the team or taking a leave of absence.  Refunds will not be issued for failure to give notice.

30-Day Notice of Cancellation form -- this must be filled out in order to suspend billing.
